Do Independent developers need to fill the vendor form?

Hi, I am new to this marketplace and about to submit my app for review. I want to implement monetization. My questions being -

i) Do independent developers need to fill the vendor application form? As the form requires Company name and Tax ID. Or do they get paid directly to their payoneer account?

ii) In case the vendor application form needs to be filled, can my personal tax info be used?
