Forms - Connected to Checklist/Other Form

I have created a work form for managers to submit employee change requests. I’d like to somehow trigger a checklist or separate form for HR to mark off certain tasks related to the change request and when the change request has been completed. What is the best way to do this?

Hey Ashley! Would subitems work to represent the tasks that HR needs to complete?

If so, you could create an automation where when an item is created (form submission), subitems are created for that item that HR can use to mark off their tasks:
