I created a form for project requests. I have a lot of display conditions. The questions appear as columns on my board, however, they no longer appear as questions in the form. They are not hidden. I don’t know where they went!
Any thoughts?

there can be a few possibilities that might cause this dependent on what view you are in:

Added Columns after Form was Live - this could be that columns were added and reordered the form entries.

Conditional Column Changes - when changes are made within the source board columns, it can sometimes change the conditional question availability.
Screenshots of what you are looking at and what views you are in would help with the troubleshooting of this for sure!
