Help with board structure

I work for a trucking company in retention. I will be calling John Doe and asking how everything is going and rating everything from 1 to 5 stars. I have 16 questions I will be asking all of this when he has been here 12 days. Then when he is here 30 days I will call back ask same 16 questions then again at 60 then 90 days. I want to be able to see if we are getting better with John Doe or worse each time.
