Items disappeared on board however still in Activity Log

Hi There,
We have recently begun using forms for our employees to fill out for Payroll purposes and have encountered an issue hopefully someone can assist with. When we have employees scanning the QR codes to enter in the details daily, their items are disappearing on the board and staying in the activity log with no way of retrieving them.
I have checked the recycle bin and they have not be deleted. Could someone please assist with why this issue is occurring?
