Inventory Check out Board

I'm trying to create an inventory check out /check in board (similar to how a library manages books). Has anyone ever done this? I've created a form for people to request items, but I am struggling on how to link that form with the actual check in/check out process. Do I need to create two boards and connect them? Thanks in advance.
