Populate a document through a form

In our use case, we have a form that leadership will have the ability to use where they will be able to submit a request to our president to make a new hire if they believe extra help is needed anywhere in the company. Right now, the "Justification" field for why that person thinks we need a new hire is routed to the item updates. Ideally, we would have almost like a questionnaire on a Monday doc that the requesting party would fill out to provide a business justification for the hire. It would be great if they could fill this out externally - then all that they have to handle is the form, and the status of the request would be managed via notifications - and in turn, the requesting party would not have to have any interactions with the board. This is not the only use case we have, we have several other processes where 95% of the board info is populated through a form, but then there is still information that is easiest to capture through a Monday doc with a template. It would be fantastic if these could be filled out outside of the board itself through a form.
