Create a Link to an Item's Conversation

On my board, an item can only be created via form. Once the form is submitted and the board’s column’s are populated, I would like a link to be created in a ‘Link’ column that when clicked will take the user to the item, preferably with the conversation window already open. I am looking for this link to be created and populated into the column automatically.

I have tried using Column Magic:Text Builder, but I’m having trouble. Below is a screenshot of my URL.
