I am creating a form to use as an anonymous survey for one of our physicians. This will be used by other physicians on our account. He is requesting that the survey is only submitted once per person. Is there a way to do this and keep the submission anonymous??

Hey Angie,
At this time, it isn’t possible to restrict form submissions on a user level, however it is possible to set restrictions on total form submissions. You can also choose to hide the resubmit button, so that users can’t resubmit after the first submission.
I understand this may not be the most ideal option, but I hope it helps in some way
