Statement in Forms in lieu of a question

Would like to be able to add a statement to a form and not just questions. For example, “If yes to all questions above, proceed to step 3” and be able to list “Step 3” as a heading and not in the form of a question. Also, we need to site laws under questions if there is a “yes” response. Would like to do this instead of creating another questions.

It sounds like you’re looking to add informative text and conditional instructions directly into the form without framing them as questions—essentially adding headers and statements to guide users based on their responses. Additionally, you’d like to reference relevant laws under certain questions if a “yes” response is selected, to avoid adding extra questions for clarity.

While Monday.com forms don’t natively support text-only statements or headers in the same way as questions, one workaround is to use custom sections to create a visual break and guide users. Alternatively, if conditional logic becomes available, it could allow for showing specific text or links based on responses.

For now, adding description fields or sub-items might also help present the guidance and legal citations you need.

Hi @Lizlombardo , We had a similar situation on my team, and here’s how we handled it. Essentially, if the user answered “No” to two or more of the questions related to their business strategy (like pillar alignment, focus areas, etc.) when raising an initiative, we wanted the form to adjust and ask for more detailed information.

What we did was add a Yes/No question: “Did you answer ‘No’ to 2 or more of the above questions?”

  • If they answered “Yes,” the form would display additional instructions and follow-up questions to gather the more targeted info we needed.
  • If they answered “No,” the form would continue with the remaining categorization questions as usual because we could reference the details in their strategy elsewhere.
