Under Permissions → Account, there is a permission called create automations/ integrations. If we select that but deselect the next drop down titled “integration” i’m assuming it will only allow automations? If so, why are these combined in the permissions tab.

You’re correct - the permission will only allow for automations if the integrations option is deselected. I understand your query here and agree that this causes some confusion including automations/automations within the same permission line.
