Hi there! I am trying to build out a performance review process in Monday and would like to have the employee and manager fill out a form that then links to this board by displaying their inputs on the master board. Is there a way to do that? Also, is there a way to only allow the individuals to see each others inputs vs other people’s reviews on the board?

At this time the form submissions will post to the specific board that it’s linked too! That said, you could set up automations or connect board/mirror columns to connect data to that high level master board.
Is this along the lines of what you’re looking to achieve?
As for permissions for reviewing inputs, our item viewing permissions might be what you’re looking for here - this is a feature that is currently available on our Enterprise plan.
