Can we create a single form for two boards?

Hi,
I need to get input through form, i have 2 boards, one is called “New Site” and other is “New Project”.
Is it possible to create a single form with condition applied for example is New site is filled it will show in site board and is new project is added it will show in project board.
please advise me solution

Hi @Aysha1 - welcome to the community!

The easiest way to complete this would be to have an automation set: When item is created and status = New Site move item to New Site board. Then you can utilize the board with the form as your New Project board.
