Looking to use a form to collect info and then create the board from templates based on the response. Additionally, want to add columns to that template based on the response.

Are you wanting to create items and columns on a board based on the form submissions you receive? If so, forms have to be created from a pre-existing board with the columns already set up that you would like to function as questions in your form.
If I am not quite understanding what you need, please let me know! I’d love to see screenshots to better conceptualize your workflow and figure out a solution. 
Hi, the use case is a person fills out a form with the products they’re ordering. We then want the board to include different columns depending on the products selected.
