Our monday form builder app, Easyform, does not add any alerts or banners to your monday forms.
Our free and starter plans only show a small logo on the bottom of the form, and our pro plan has no branding at all. See below for an example of our form in action.

Hi Ian. It might depend on your account level, but on Enterprise you can - when editing the form, under Customize / Submission View there’s a Hide Branding option. If you don’t have that due to being on a lower account tier, I don’t think you can remove it.
Under Customize / Submission View, there’s an option to Hide Branding. If this option isn’t available due to your account tier, you may not be able to remove it.
