Creating Workspace for receiving information from website form

I have created a Zapier account to integrate my website Ninja Form with Monday and am trying to get all form submission contents upon submitting show up over in my Monday account rather than to my GSuite email account. I have created a new Workspace to accomplish this on Monday and it appears my Zapier Zap is communicating successfully with Monday, however, when I do a test from my website form, non of the specific information that gets filled out (Name, email, city, message) shows up over on Monday. All I get is an added item called ‘New Form Contact’ but nothing beyond that.

Any help on what I need to build over on Monday to show everything would be greatly appreciated. Thank you!
