Generating a Form to fill in upon status change

Hi there,
I’m trying to find a way to generate a form that needs to be filled in when the status of an item on a board is changed to “Done”.
For example
Status changes to “Done” - Form pops up asking for details on what was done - those details are then populated into a column on on the board.

Hi Casey, I have a similar requirement from a feedback form perspective. Did you have any success with this?
