Hi there. Currently have a team of 25 who all use Monday. Within the team, there are 3 sub teams who all have there own individual request forms, asking their own specific questions to gleen information that then creates a job card. We want to combine the three forms into one - so we can have the same link as opposed to three.
Need an initial screen that will ask visitors is your request A, B or C? when they click B, it takes them to Team B’s form, Click A, team A’s etc
Is this possible and if so, how can I go about implementing
Definitely! You’ll just use Conditions on the Workform. Basically you’ll tell it 'IF they choose Team A, then ask them ‘these questions’, if they choose Team B, then ask ‘those questions’ etc
If you need a more complex form, I found that Fillout.com has a great form builder that integrates seamlessly with Monday.com and they have a free tier that is great.
Are you looking to combine all three forms into a single form that creates items in a single board, or a directory that points at three forms creating items in three different boards?
If what you are looking for is a directory, you can create a page on your website with three buttons, each pointing to a form, and then share the link to that page.
