Can you set up an automation that when the status changes to something it will add the item to another board, but only if the item name is not already there, so you are not creating a duplicate item?

Welcome to the community. You can use the automation shown in the attached image together with the Duplicates and Uniques app to achieve what you are trying to do
Basically, the automation will create the item in your preferred board when the status changes. Then in that board, you can set up a template which will automatically check your items for duplicate item names and delete the item if the item name already exists.
