I believe it may not be possible to achieve this functionality solely using Monday.com. You might need a third-party tool to capture the date and time during the scan. This will be feasible only if you have a solid understanding of APIs, as your question indicates a requirement for very specific data.
Create automation for creating QR codes. You can name QR code whatever you like and select single-use or multi-use based on your use case (single scan QR codes can be used only once)
Create a custom automation using trigger introduced by the QR Flow app. As an action set “set date” action (monday.com native action). The name must much the name from the previous automation.

I have the exact solution for this using QR Item Tracker on monday.com - it automatically record scan time when a QR Code is scanned.
